Review of Systems School of Medical Transcription Enrollment Agreement
I would like to enroll in the Review of Systems' Beginning Medical Transcription course. Enclosed is a copy of my high
school diploma or equivalent. I understand the cost of the course is $1995 ($1220 for course materials, $665 for tuition,
and $120 for shipping). Upon successful completion of the course (a grade of C or above), I will be awarded a
Certificate of Completion. Upon successful completion of this course with a GPA of 95% or better, I will be guaranteed
a job through one of Review of Systems’ work partners. All materials provided in the course, except for test tapes or
CDs, are mine to keep. I understand an instructor will be available via an 800-number during regular business hours (9-
5 Eastern Time, Monday through Friday). I must complete the course in 18 months, unless sufficient reason for needing
more time to complete the course is provided in writing to the school. I understand I will complete the 10 test tapes,
with one test to be submitted for correction at least every 8 weeks. These 10 tests are considered “graded assignments”
and may be taken without supervision. A final exam will include a 15-minute test tape and two-page written test. This
examination will be proctored (supervised by a party agreed upon by the school), and I will be able to use all available
references, resources, etc. With each test tape, a grade and correction sheet will be returned to me promptly. If I do not
successfully complete each test with a 70% or above (I will be allowed two attempts at each test), I will be terminated
from the course and refunded tuition according to the refund policy stated herein. (A sample copy of the correction
sheet, which explains how grades are calculated, how many points are deducted for each type of error, etc., is available
from the school upon request. A copy is also provided in the study guide.)
Refund Policy
Upon receipt of program, I have twenty (20) days in which to look the materials over. If I decide to cancel my
enrollment, I must return all materials in unused condition within that 20 days to receive a full refund (less shipping). I
understand that this 20-day period cannot be extended for any reason. I understand I must withdraw in writing. I
understand, if I withdraw from the program after 20 days, all materials are mine to keep. Students purchasing the course
by sections are not obligated to purchase additional sections nor do they qualify for tuition refunds.
Students who have purchased the entire program and have remained in good standing with the school (see "Termination
Policy" below for good standing requirements), can withdraw from the course at any time and be refunded money for
tuition on the following schedule:
Before completion of any test tapes but not more than 16 weeks after start date: 100% of paid tuition will be refunded
After completion of test tape 1 but not more than 16 weeks after its completion: 75% of paid tuition will be refunded
After completion of test tape 2 but not more than 16 weeks after its completion: 50% of paid tuition will be refunded
Termination Policy
I understand a test tape must be submitted to the school at least every 7 weeks in order to maintain good standing.
I understand if I do not successfully complete each test within the allotted time and with a grade of 70% or above, I will
no longer remain in good standing and as such will be terminated from the course. I further understand that in such case,
I am entitled to keep all materials received to the date of termination, except for test tapes.
I understand, if I am unable to complete a test within the allotted time due to health problems, family crisis, etc., I can
be granted an extension for an additional 16 weeks. I understand that any such extension applies only to my ability to
remain in the program and does not extend the time in which I am entitled to a refund of tuition. The request for such an
extension must be submitted in writing and no more than one such extension shall be granted to any student.
Additionally, if a student completes a test but receives less than the required 70%, that student may be granted one
additional attempt to complete that test to achieve the required grade.
