DERMATOLOGY PROOFREADING EXERCISE

HISTORY OF PRESENT ILLNESS:

Mrs. Smith is a farmer patient of mine now referred back for her regular skin cheek. She does have a history of actenic keratoses, which has responded well in the past to liquid nitrogen. As well, she has had recurrent peri-oral dermatitis, which is clear at this point.

PHYSICAL EXAMINATION:

On examination today, the patient has prominent actinic keratoses on the right shoulder, right back, and dorsal hands. Of note is the face that the lesion on the shoulder is somewhat thickened and enflamed.

ASSESSMENT:

Actinic keratoses.

PLAN:

All lesions were treated with liquid nitrogen today. The patient will start using Retin-A .025 verses Retin-A Micro on the sun-damaged areas of her skin. She will come back to see me in one months' time if the thicker shoulder lesion has not resolved, at which time it can be biopsied.
